your name and phone number, your agency / organization name, address and phone number (if different from previous), and number of copies. The DVD(s) will be sent upon receipt and processing of payment; allow 7-10 days.

within the purchasing agency / organization for staff training,
periodic 'refresher' training for staff, training of new staff, etc.

with travel and overnight costs totaling approximatley $1500.00 per
training. A 1/2 hour teleconference consultation is available for the first time use
of the DVD within six weeks of purchase. Make this request via email link below with date of
